Lacey Strong Coaching Terms of Service

By enrolling in coaching services provided by Lacey Pruitt and Team LaceyStrong, LLC, I certify and acknowledge the following:

1. Independent Trainer & Assumption of Risk

I acknowledge that Lacey Pruitt and associates (operating under Team LaceyStrong, LLC) are independent personal trainers and nutritional advisors. I have been informed that participation in cardiovascular and resistance training programs, as well as diet plans, may result in physical injury. Any pre-existing conditions or health concerns must be disclosed to Lacey Pruitt prior to or during training. Failure to disclose relevant health information is solely my responsibility.

2. Release of Liability

I freely and knowingly assume all risks associated with participation in these programs. I hereby waive any right, claim, or cause of action against Lacey Pruitt and Team LaceyStrong, LLC, and release them from any liability for injury, illness, damages, expenses, or claims arising directly or indirectly from my participation in any training, nutrition plan, or competition prep.

3. Right to Terminate Services

I understand that Lacey Pruitt reserves the right to terminate my coaching plan and/or competition participation at her discretion due to concerns regarding my physical or mental health. This decision is made solely in the interest of my well-being.

4. Non-Medical Professional Disclaimer

I acknowledge that Lacey Pruitt is not a licensed medical or nutritional professional. Any guidance provided is based on personal experience and industry knowledge and should not be considered medical advice.

5. No Refund Policy

I fully understand and accept that once I receive my initial plan, all payments are non-refundable under any circumstance.

6. Agreement to Terms & Digital Signature

I confirm that I have read and agree to this Liability Waiver and Terms of Service. I acknowledge that submitting my printed name and payment constitutes my signature and agreement to all terms. I also confirm that all personal information provided is accurate and truthful.

7. Three-Month Minimum Commitment

I acknowledge that coaching services require a minimum commitment of three (3) months. If I choose to terminate services before completing the three-month period, I am still financially responsible for all remaining payments.

8. Confidentiality & Non-Distribution Agreement

I understand that all coaching programs, training plans, and materials provided by Lacey Pruitt are proprietary intellectual property. I agree not to reproduce, share, distribute, or disseminate any materials during or after my coaching relationship without prior written consent from Lacey Pruitt. This confidentiality agreement remains in effect indefinitely, and any unauthorized sharing may result in legal action.

9. Long-Term Service Agreement

I acknowledge that this agreement remains active until terminated in writing by either party. Termination is subject to the conditions outlined in this contract.

10. Authorization for Automatic Payments

I authorize Lacey Pruitt and Team LaceyStrong, LLC to charge my credit/debit card on file for all coaching fees as per the agreed-upon payment schedule.

11. Automatic Subscription & Cancellation Policy

All charges for coaching services operate as automatic recurring payments based on the agreed subscription schedule. It is my responsibility to submit a written cancellation request if I choose to discontinue services. Failure to submit a cancellation request will result in continued charges.

12. Additional Terms & Acknowledgements

• I understand that results are not guaranteed and are based on individual effort, consistency, and adherence to the program.

• I confirm that I am of legal age and mentally competent to enter this agreement.

• Any disputes arising from this agreement shall be resolved under the laws of the governing jurisdiction where Lacey Pruitt operates.

By proceeding with payment, I acknowledge that I have read, understood, and agreed to all terms stated above.
